I have a spousal visa for south africa, due for renewal in September. Do i need all the things i got before when i applied for the visa (marriage cert, police clearence, medical report, lung xray, letter from spouse, etc) or do i just take my passport and visa to home affairs? and what form would i need? There are forms you need to fill out and all the paperwork needs to be in the format required by the authorities.
When I was applying for my visa I had to get the forms from Home Affairs as they weren't available online. However, since then things have changed quite dramatically. Now VFS has taken over the processing of visas, therefore you should visit their website for info on relavent forms, etc

I suggest using the link above. It should explain the process in quite some detail and tell you whether you need to visit the embassy in the UK.
The changes that took place were quite extensive and many people were forced to return home to apply for visas. I know that in SA home affairs are no longer accepting applications but VFS are handling them. I would approach the UK or embassy or VFS directly to check how to go about it though.

Yes you generally need all the same documents you used the first time around. Some documents like medical certificates, etc - you won't need to rebmit. Just include a note saying you have previously submitted them. You should visit the home affairs website to get a comprehensive list of exactly what you need.
In April/May 2014, a lot of changes were made to visa legislation and although this shouldn't really effect you if you are married - its still worth taking a look into.
Get the ball rolling asap so you allow time for any delays.

There have been lots and lots of changes in recent months and the DHA is no longer taking applications and these are now dealt with by VFS.
